Heartworm Transmission

Heart worms are about six inches long. They live mostly in the heart and the large blood vessel that bring oxygen-rich blood from the lungs to the heart. Adult male and female worms living in the heart produce thousands of microscopic baby worms which circulate throughout the body. These baby heartworms do not grow to adulthood in the dog where they were born. (If they did, the dog would quickly die and that would be bad for both the dog and the heartworms.) Before baby heartworms can develop further, they must find their way to a mosquito.

So then, a mosquito comes along and bites the infected dog, sucking up baby heartworms. This probably isn't too good for the mosquito, but it is what the worms have been waiting for. During the next month, the heartworm babies develop into heartworm teenagers, a stage partway between baby and adult.

Now, the mosquito bites another dog, infecting the new dog with teenage heartworms, ready to develop into adulthood. After six or seven more months the life cycle is complete: new adult male and female heartworms are producing thousands of baby heartworms.

Heartworms in the Northern Sierra foothills The tree hole mosquito, which breeds in oak trees at our elevation, is very good at spreading heartworms. It lives longer than most mosquitoes, takes repeated blood meals, and even seems to remember where it found food the last time. Since the mosquito remembers where it got a good meal last time, it is likely to deliver teenager heartworms back to the to the same dog they originally came from as babies or miss that particular dog and give them to the dogs that live next door. As a result we often see clusters of very severe heartworm infestation.

In 1972, when the heartworm problem in this part of the United States was first being recognized, 25% of all heartworm infections which were reported in the state of California that year were reported by our hospital. Entomologists from U. C. Berkley came to Placerville to catch heartworm-infected mosquitoes for their studies.

We have a lot of heartworms here. Unprotected dogs living outdoors will almost certainly catch heartworms. Indoor dogs going outside in the morning and evening will probably catch heartworms. Statistics show that long hair offers no protection whatever.

Canine heartworm disease & canine heartworm symptoms There are no symptoms at all until the disease is very advanced. Then, the symptoms are those of congestive heart failure: dull coat, lack of energy, coughing, difficulty breathing, perhaps fainting spells and an enlarged abdomen. Waiting for symptoms to develop is not a realistic alternative to prevention.

Canine heartworm prevention There are several different drugs used for heartworm prevention, all of them highly effective and easy to use. You have your choice between a monthly good-tasting pill, a monthly application to the skin or a once-ever-six-months injection. All are highly effective and cost about the same, although there is a new semi-generic product that's a bit cheaper. In cold climates, prevention medication is unnecessary during the winter. In Placerville, we have mosquitoes year 'round, and although we could theoretically skip the medication when it is sufficiently cold, the weather is different every year. Our recommendation is once a month, year 'round, permanently

Canine heartworm testing No medication is perfect, and none of us have perfect memories. Dogs should be periodically retested. In situations where pet owners feel confident that the medication is being given regularly, testing every two years is adequate.

Heartworm treatment in dogs Although heartworms can be fatal, and treatment for the disease involves risk, the condition is nearly always curable. Treatment requires careful medical care and complete rest at home afterwards. See heartworm treatment in dogs.

Feline heartworm disease Because cats are not a natural host for the parasite, heartworm disease in cats is a very different problem. Cats are much less likely to get heartworms than dogs are. When they do, symptoms are unpredictable and seemingly illogical.

Heartworm symptoms in cats Usually there are no symptoms. When noted, symptoms resemble those of other more common problems.

Heartworm treatment in cats We do not have a satisfactory drug for heartworm treatment in cats. If we did, using it would be risky because cats do not deal well with dead heartworms. They sometimes absorb the dead worms successfully; they sometimes die from arterial obstruction or from allergic reaction to the dead worms. So, if we could kill feline heartworms, it is not clear that we should. As it stands now, the best treatment for feline heartworm disease is to start on prevention (so they don't get more) and try to control the symptoms, if there are any.

Heartworm testing in cats Primarily because of the low heartworm numbers found in cats and the fact that heartworms do not reproduce in cats, testing is much less reliable than in dogs. Although the American Heartworm Society considers it "good medical practice" to test cats before starting on prevention and it is hard to argue with this, we feel that effort and expense required to test the entire cat population would be better devoted to doing something more useful. After all, even if we know that a cat has heartworms, our treatment is exactly the same as for a cat that doesn't have heartworms: give medication to keep form getting more heartworms and watch for symptoms.

If your cat has unexplained vomiting problems, or especially if there is difficult or noisy breathing, heartworm testing is an important part of the diagnostic workup. If you "just want to know", that is also a good reason to test for heartworms. However, we do not ordinarily test cats that look good and act normal.

Heartworm prevention in cats We urge everyone in our area to protect their cats from heartworm disease. If your cat is entirely indoors and you don't get mosquitoes in the house, she may not need heartworm protection, although a surprisingly high proportion of feline heartworm cases occur in "indoor" cats.

Heartguard FX for Cats is a once-a-month, good tasting palatable kitty treat medication. Most cats enjoy it or accept it readily mixed with their food. If necessary, Heartguard can be given like a pill. More often, we recommend Revolution, which controls heartworms, intestinal worms, fleas and ear mites and is also easier to use.

HEARTWORM STUDY IN OVER 51,000 DOGS Heartworms are large nematode worms that live in the heart and major arteries of dogs. The adult worms produce larvae that circulate in the blood stream. The larvae are picked up by mosquitoes when they feed on infected dogs and are then transported by the mosquitoes to new dogs. Until the early 1960s, heartworm was known in America only in the Deep South. In the 1960s, it spread up the Mississippi Valley and now occurs throughout the United States wherever mosquitoes are present and temperatures are sufficiently warm.

Recently, the University of California at Davis conducted a study in which over 51,000 dogs in Northern California were tested for heartworm infection. The incidence of heartworm in San Joaquin county was very low, with one positive dog for every 40 to 100 dogs tested. However, infection rates in Amador and El Dorado counties were high, with one positive dog for every 6 to 20 dogs tested. Infection rates were also high in parts of the Sacramento Valley (especially in the north) and in coastal valleys where moisture and temperature conditions are favorable to mosquitoes.

Because the heartworm infection rate is increasing and due to the proximity of areas with a high rate of infection, we recommend that all dogs be on heartworm preventative. Prior to initiating preventative medication, dogs should be tested for heartworm to establish that they are not already infected.

Ringworm and public health issues

Despite its name, ringworm is not a worm. It is a fungal infection of skin. So "worming" your pet will not protect against it.

Public health issues: The ringworm fungus can spread to humans, especially children It causes a circular red "ring-like" lesion on the skin surface (hence the name). If you suspect a ringworm lesion, see your doctor ASAP for treatment as it can be very difficult to remove if it gets into the scalp. Transmission: Ringworm spreads from pet to pet either by direct contact, or by sharing infected bedding. Symptoms Ringworm skin lesions look like circular areas of hair loss. They can appear anywhere on the body. Favourite sites are the face and toes.

Diagnosis A vet can run several tests to diagnose ringworm. Approx. 70% of ringworm lesion glow yellow-green under an ultra violet (UV) light As skin scrape may reveal ringworm fungal elements when viewed under the microscope (see images below) If there is no result with the UV light, a culture of the suspect hairs is made.Treatment • Long haired pets need to be clipped • All bedding should be removed as it may contain infected hairs.

• Use Malaseb shampoo 2-3 times a week leaving it on for 10 minutes each time. • Traditional therapy involves giving a drug called Griseofulvin. However, it is not without slight risk of causing "dry eye", a condition where the eyelid glands stop making tears. •

Program paste given orally is very effective at killing ringworm. The dose for cats is approx. 100mg/kg repeated 2-3 weeks later. We tend to use Program paste instead of Griseofulvin. • All infected and in-contact animals need to be treated

WORMING - Your Dog for Tapeworm Control:

General Information

Tapeworms can conjure up many images and thoughts of a ghastly infestation. However, in comparison to other intestinal worms it is probably the parasite with the least direct health problems. Although they compete with the infected pet for the nutrients of ingested food, they do not generally harm the host. Tapeworms, which include several types, reside in the intestines of dogs, cats, humans, rats, mice, fish and others animals. They consist of a head and a long flat body made up of segments. This body can be several feet, if not more, long. Segments, each having the ability to reproduce, are passed in the animal's feces, leaving the head still attached to the animal's intestinal lining, where it produces new segments. One of the worst case scenarios is that if left untreated it may become so long that it obstructs the intestines.

As a general rule though, tapeworm infection may not cause any noticeable illness in your pet, or it may show moderate signs of infection. These symptoms may produce digestive upsets, excessive or decreased appetite (depending on the pet), poor hair and skin coat (possible balding, red areas especially on the rump and feet), weight loss and vague signs of abdominal discomfort. But, what most owners who've had a pet with tapeworms can tell you is that the first thing you most likely notice is the 1/4 inch, whitish worms that may expand and contract in your pet's stool. Or the cucumber seed-like or rice grain-like dried segments in their pet's bedding area or clinging to the hair around their butt.

However disgusting it may be finding one of these segments, this is the best form of diagnosis there is since the eggs may not be found upon microscopic examination of the feces by your veterinarian. The reason being is that the nature of the worm's ability to carry the eggs inside the segments and are not laid in the stool as are other worm's eggs, which is how most intestinal worms are diagnosed.

The good news is that tapeworms are not passed directly from pet to pet, but require an intermediate host in which to develop. It is not transmitted by direct contact but by ingestion of an intermediate host either while eating or grooming. The bad news is that the common intermediate hosts are fleas and small animals, such as mice, rats, squirrels and rabbits. Fish are the intermediate host for one type of tapeworm, but this type is only occasionally seen in domesticated pets.

Treatment

Treatment for the most common form of tapeworms in pets can be treated by a single or oral dosage DRONCIT. There are several different types of dewormers available. Recent studies, stated by Ohio State University, have shown that the efficacy rate is better in oral medication DRONCIT, As you may know, some animals especially cats, just don't like taking pills and the quantity of pills needed for a large dog increases the expense. This SINGLE dose of RDONCIT will destroy the tapeworms within 24-48 hrs that are in the body at the time of dosage. Since the eggs reside in the segments itself, they are all killed along with the segments.

Now, this does not mean that a pet will not get reinfected. In fact, if you do not eliminate the intermediate hosts (fleas, etc...) than you may experience the whole cycle again in as little as two weeks.

Control of the environment is the key

1. Fleas must be eliminated from both the pet and the environment, preferably before bringing them home from the vet.

2. Apply a flea control to your pet - Advantage or Frontline Plus.

3. Don't forget to treat the inside and outside, especially the bedding area with a flea control.

4. If your pet's a hunter at heart, don't allow them to eat small rodents or raw fish.

5. For maintainence, use a fine tooth flea comb to inspect for infestation and to destroy stragglers. Remember, even one infected flea can cause tapeworms. You may find it useful to dip the flea comb in rubbing alcohol to stun the fleas. Though this may not be advised for sensitive skin.

Tapeworms are a frustrating and disgusting ailment because fleas are usually involved.

WORMING - Your Dog for all intestinal worms:

Puppies should be wormed every 2 weeks from about 4 weeks of age till 12 weeks, then monthly until 6 months of age, then as for adults once every three months.

Adults: Please worm every 3 months. They are wormed against Roundworm, Hookworm, Whipworm, Tapeworm. Roundworm and Hookworm larvae can infect children.

While not a major health risk, Dipylidium can cause itching around the dogs hind end leading to scooting and/or anal gland problems. Hydatid Tapeworms, Echinococcus granulosa, are more serious. They are found in areas where dogs and sheep are in contact and pose a serious health threat to humans.

HEARTWORM:

This is a disease due to a large worm that affects the heart and adjacent blood vessels. It is carried by up to 60 species of mosquitoes. Treatment of this potentially fatal disease is quite involved and expensive, but PREVENTION is as follows:

Daily dosing with D.E.C. throughout the year, eg. Dimmitrol.

Monthly dosage of a drug throughout the pet's life. These are more effective at breaking the parasite's life cycle, eg.,

Heartgard - heartworm only. Used in combination with Drontal for total worm control.

Interceptor - heartworm plus roundworm, hookworm, whipworm control.

Sentinel - this product combines Interceptor with Program (see external parasites) and thus provides a degree of Flea control.

Proheart - available as Monthly Tablets.

Advocate and Advantage Duo or Revolution - applied monthly for heartworm and fleas.

Fleas: Control and elimination depends on treating your pet and its environment. Our recommended regimes are as follows:

Frontline Plus - SAFE and effective for about 4 weeks - highly recommended, even on pups - Frontline Plus on pups from 12 weeks age.

Ticks These are obligate ectoparasites of many terrestrial vertebrates worldwide. They resemble mites and like mites they belong to the subclass Acari; however, they are considerably larger than mites. Infectious species - A small number of species of two of the three families of ticks, Ixodidae (hard ticks) and Argasidae (soft ticks) occasionally parasitize dogs and cats. These two families contain more than 800 species. Ticks are called hard because the dorsal covering of the male is hard. Although the two families share some basic features they differ in many others including morphologic, physiologic, feeding habits and reproductive patterns. Members of the family Ixodidae are by far the most common ticks found on dogs, cats and other domestic animals. Important features of ticks - They can be readily seen with the naked eye. The unfed larvae are about 1 mm in length and just visible. There are one, two and three host ticks. One host tick - the entire development from larva to adult takes place on one host. Two host tick - the larva and nymph occur on one host and the adult on another. Three host tick - each stage of development, larva, nymph and adult takes place on a different host. The life cycle can take as long as three years to complete. "Ornate" ticks have a colored enamel-like area covering the body. Ticks are temporary parasites of many animals; they stay on the host for short periods. The adults of common hard ticks (Ixodidae) can live for months off the host; some up to two years. Many tick-borne pathogens remain in the tick when it molts from one stage to the next, e.g., Ehrlichia canis in Rhipocephalus sanguineous. This is called transtadial or horizontal transmission. One host ticks and some three host ticks exhibit transovarian transmission of pathogens, i.e., they infect the ovaries of the tick and pass with the eggs to the next generation of ticks, e.g., the rickettsia of Rocky Mountain spotted fever in Dermacentor andersoni and D. variabilis. As indicated below, a number of important canine, feline and human diseases can be transmitted by ticks. Life Cycle - The life cycle of three host ticks is briefly as follows.The fertilized, engorged female drops off the host (1) and lays eggs, usually more than a thousand. Eggs hatch yielding six-legged larvae (seed ticks) that live on vegetation, e.g., grass and shrubbery. They attach to an appropriate host (2), feed for several days, drop off and molt into eight-legged nymphs. These attach to an appropriate host (3) and after a blood meal, drop off and molt into an adult. Clinical Features - Ticks are found more commonly on dogs than on cats. They are generally of little clinical significance in either animal. The most common sites of infestation are the head , neck and ears, the less accessible areas for scratching and grooming. Ticks can produce some inflammation of the skin and possible allergic reactions at feeding locations. The larvae and nymphs of Otobius megnini cause a severe dermatitis in the external ear that is characterized by a waxy exudate. Heavy infestations on rare occasions lead to anemia. Tick-bite paralysis, which is an important complication, is discussed separately below.

Some Important Ticks Parasitizing Dogs and Cats in North America Rhipicephalus sanguineus - This inornate, three host tick (dog ?dog ?dog), referred to as the brown dog tick is found worldwide and is primarily parasitic on dogs. It transmits canine babesiosis, canine monocytic ehrlichiosis, canine haemobartonellosis and may produce tick paralysis. Dogs can contract hepatozoonosis by ingesting R. sanguineous. Dermacentor variabilis - This ornate, three host tick, referred to as the America dog tick, occurs in the Eastern and Central US, Eastern Canada and Mexico. It causes tick paralysis in dogs most commonly and occasionally in cats. It transmits Rocky Mountain spotted fever, tularemia and is suspected as a vector of feline cytauxzoonosis and canine granulocytic ehrlichiosis. Dermacentor occidentalis - This ornate, three host tick, the so-called Pacific coast tick is mainly found in coastal areas of western US. It is a three host tick that may be found anywhere on the horse. It can transmit Rocky Mountain spotted fever to humans and dogs, Colorado tick fever to humans, tularemia, and causes tick paralysis in humans, dogs and cats. Dermacentor andersoni - This ornate, three host tick, called the Rocky Mountain wood tick, is found in the western US and Canada. It transmits Rocky Mountain spotted fever and tularemia and causes tick paralysis in dogs and cats. Ixodes scapularis (dammini) - This inornate, three host tick, referred to as the shoulder or deer tick occurs in North America. It and I. pacificus transmits Lyme disease. Amblyomma spp- Species of this genus are not very host specific and some are found on dogs and cats. Amblyomma americanum - This, the so-called lone-star (silvery spot on female) tick, has three hosts and occurs in the South Eastern to South Central US and in tropical Latin America. It transmits tularemia, Q-fever, Rocky Mountain spotted fever, Lyme disease and is suspected as a vector of canine granulocytic ehrlichiosis. Amblyomma maculatum (Coast tick) - Tropical Latin America and Atlantic and Gulf Coast. It transmits Hepatazoon americanium in the Americas. Amblyomma cajennense- Occurs in tropical Latin America and south Texas. It transmits Rocky Mountain spotted fever. Otobius megnini - This argasid, soft tick, is found in the ears of dogs and cats in the Americas and parts of Africa and Asia. Only the larvae and nymph forms are found in the ears. The adults can survive on the ground for as long as two years. Diagnosis - Ticks and particularly the larger engorged females, can be readily seen on the skin. A number of morphologic characteristics are used by parasitologists to identify the various species. Treatment -

Dogs and cats that have been in areas where ticks are prevalent should be carefully examined and ticks carefully removed. To remove, the head of the tick, which is usually buried, is grasped firmly with forceps as close to the skin as possible and pulled straight-out. Preventive flea treatments are effective; the treatment for dogs differs from that for cats. Various acaricides including organophosphates and pyrethroids are used for dipping and application to the environment. Flea or tick collars are used and acaricidal preparations are applied to ears to control ear ticks.

Control -

Keep dogs and cats from exposure in tick infested areas. Spray kennels and environs with acaricides. Prevention of tick infestation is particularly important in that when the tick or ticks are found on the host a disease such as canine babesiosis or hepatozoonosis may have already been transmitted.

Public Health Significance - Ticks from dogs and cats occasionally bite humans with the potential of exposing them to the tick borne diseases, Lyme disease, Rocky Mountain spotted fever, Q-fever, human babesiosis and relapsing fever.

Tick Paralysis (Tick-bite Paralysis) More than twenty species of ticks with varying geographic distribution have been reported to have the capacity to produce tick paralysis. This condition mainly affects humans (children especially), dogs (occasionally) and cats infrequently. It is assumed to be due to a protein neurotoxin produced by ticks and introduced during biting. Those mainly involved in North America are D. variabilis, D. andersoni, and D. occidentalis. In Australia, Ixodes holocyclus produces a particularly potent neurotoxin. Young animals are most susceptible to tick paralysis. Toxicity - The circulating neurotoxin interfers with acetylcholine release at neuromuscular junctions leading to an ascending paralysis. Toxicity is mainly associated with female ixodid ticks and incidence of the disease is related to the latter's activity in the spring and early summer. The incubation period is 5 - 9 days, i.e., from the time of introduction of toxin until signs appear. Clinical Features - Temperature is usually normal. Initially there are discomfort and often vomiting and dysphagia. Abnormal fore- and hindlimb reflexes, unsteady gait, incoordination and weakness with ascending paralysis. Sensation and consciousness is retained. In a few hours the animal is unable to move, breathing is difficult and death may ensue in several hours if ticks are not removed. If ticks are removed in time recovery takes place within 1 - 3 days. Diagnosis - This is based on the progressive paralysis and the presence of ticks. Rapid recovery after tick removal is confirmatory. There are no observable lesions and blood and fluid values are normal. Treatment and Control -

There is none, other than the prompt removal of ticks. Measures should be directed toward reducing exposure to ticks and to their removal when present.

Public Health Significance - Humans are susceptible to tick paralysis initiated by tick bites; however, animals with tick paralysis cannot transmit the disease to human

DESEXING:

If you do not intend to breed from your pet then this procedure is recommended, particularly if you live in an urban area. It is perfectly satisfactory to desex the female pet before her first season. Our recommended age for both sexes is about 6 MONTHS old. This is a good time to consider MICRO-CHIPPING as a form of permanent I.D for your pet.

Sarcoptic Mange

Sarcoptic mange is a very itchy disease caused by a small mite not visible to the naked eye. Areas where the mites tend to burrow under the skin include the tips of the ears, elbows, hocks, chest and belly. However, in a severe infestation, mites can cause problems on the animal's entire body.

Diagnosis is made by performing several skin scrapings and examining the debris under a microscope. Unfortunately, in many dogs that have sarcoptic mange, no mites can be seen on skin scrapings. If sarcoptic mange is suspected, treatment may be the only way to diagnose and cure your dog.

This disease is highly contagious and can spread from pet to pet or from pet to human. Other dogs in the household should be treated whether or not they have symptoms. If any members of the family have red itchy bumps on their skin, they should consult their physician. Mites can only survive a short time off the dog; however, to ensure that there is no reinfection, your dog's living quarters should be thoroughly cleaned and bedding washed or thrown away.

No matter which treatment is chosen, your dog may remain contagious for the next 2 to 4 weeks. Please keep him/her confined and away from other dogs and unexposed persons until you re-check appointment.

Treatment

Treatment sometimes includes clipping your dog's hair coat. This will depend upon length of the hair coat and severity of the disease. If a dip is used, a good shampoo to remove all the scales and crust before dipping is recommended.

Several medicines can be used to kill the mites. These are in the form of dips, oral medications or spot-on products.

Lime Sulfur Dip (LymDyp®) Shampoo your dog with a medicated shampoo before each dip. Dip all in-contact dogs with the diluted lime dip. Avoid getting the lime sulfur on paint, wood and jewelry while dipping your dog. Unfortunately, the lime sulfur will smell like rotten eggs. It may stain porcelain tubs. Do not rinse the dip off, the smell will dissipate with time.

Ivermectin (Ivomec®) Ivermectin is very effective at killing mites. It is used as a heartworm preventive. For scabies, it is widely used, but the FDA has not approved its use in the dog at the necessary dose. Thus it is an experimental treatment for scabies. With your approval, your dog will be given Ivermectin Valueheart treatments every 14days. This treatment is never used in collie-type herding breeds.

Selamectin (Revolution®)

This is an endectocide that is effective against many parasites including the Sarcoptic mange mite. It is placed on the skin at the back of the neck and absorbed into the system to have its effect. It takes two doses, one month apart, to have 100% success. Use a vial of Revolution on you dog or cat acording to body weight.
